Pilot mismanagement to blame for Asiana crash

The US National Transportation Safety Board says last year's crash of Asiana Airlines flight 214 was mainly due to the faults of the pilots.

Calling it the line-up of a series of unfortunate events, acting NTSB Director Christopher Hart cited many concerns, including the crew's over-reliance on automated systems. The NTSB said the crash was caused by the pilot's mismanagement of the descent of the plane - a Boeing 777.

The crash killed three passengers and injured more than 180 others.

Asiana said in a report to the safety board that the crash likely was due to the pilots flying dangerously slow and an inadequate warning system that should have alerted them.

The low, slow landing caused the plane's tail to hit a seawall short of the runway. The plane spun 330 degrees as it broke apart and caught fire, strewing wreckage along the runway.
